Once these items are obtained, apply for a permit for your pet to return to the U.S. at this link. For further guidance on returning to the U.S. with your pet from a high risk country, contact the CDC directly at cdcanimalimports@cdc.gov
- Request an import permit from Animal Quarantine Station (AQS) via email at moacdc@thaiembdc.org or qsap_bkk@dld.go.th
- Send the completed permit, attached R1/1 form, a copy of your passport, a photo of your pet, flight itinerary, vaccination record in English and a certificate of microchip to a certified veterinarian to be accredited and the endorsement office in the country of origin.
- After these steps are completed, a certificate will be issued that is valid up to 10 days before travel.
